Value Lever

A multinational pharmaceutical company with $2B in revenue sells life-saving medicines in 120 countries. This pharmaceutical anti-counterfeiting case study shows how it cut counterfeits in circulation by 84% across 4,200 SKUs in 52 countries, without adding packaging material or changing a production line.

Problem

The company sells in 120 countries and runs direct operations in 52 of them. Counterfeits carrying its brand were circulating through unregulated online pharmacies and gray-market channels, putting patients and a reputation built over decades at risk.

At the same time, it had committed to cutting its carbon footprint 40% by 2030, digitizing patient information and removing printed materials.

Those two goals pulled against each other. Traditional anti-counterfeiting features, such as holograms, barcodes, security inks and special labels, all add packaging material. Ennoventure's invisible cryptographic authentication met both goals at once.

The Critical Problem

Three kinds of counterfeit

  • No active ingredient: packs that look genuine but contain no active pharmaceutical ingredient (API), so patients go untreated without knowing it.

  • Substandard API: wrong doses or degraded ingredients that cause treatment failure or adverse reactions.

  • Toxic adulterants: dangerous substitutes or contamination that can cause serious harm or death.

All three were packaged to look identical to the genuine product.

Channels at risk

Unregulated online pharmacies posed as legitimate vendors, offering convenience, privacy and lower prices. To an ordinary patient, their websites and payment systems looked no different from a real pharmacy.

What one counterfeit incident costs

  • Patient harm from toxic ingredients or treatment failure

  • Lost trust: only 26% of patients exposed to a counterfeit stayed with the brand

  • Regulatory action, up to licence suspension

  • Incident costs averaging $8.4 million, plus litigation averaging $12.6 million

  • Five to seven years to rebuild the brand's reputation

  • Patients switching to competitors

Scale of the threat

The WHO estimates that about 1 in 10 medical products in low- and middle-income countries is substandard or falsified. Before deployment, the company estimated that 8.2% of its products in circulation were counterfeit. That meant:

  • Around 300,000 counterfeit units in patients' hands each year (range 240,000 to 360,000)

  • Around 222,500 patients at risk each year (range 178,000 to 267,000)

The Challenge: Solve Without Sacrificing Sustainability

The pharmaceutical company faced a dilemma: traditional anti-counterfeit solutions (holograms, embossed labels, special inks, multi-layer security packaging) required:

  • Additional plastic or composite materials

  • Specialized printing equipment

  • Extended production timelines

  • 8-12% increase in packaging weight

All contradicted their sustainability commitment to reduce carbon footprint and eliminate unnecessary printed materials by 2030.

The company needed authentication that was:

  • Invisible (no additional materials)

  • Scalable (across 4,200+ SKUs globally)

  • Fast-deploying (no production line changes)

  • Patient-accessible (smartphone-based, multilingual)

  • Data-driven (real-time counterfeit intelligence)

  • Sustainable (reduces, not increases, environmental impact)

The Solution

Ennoventure combined invisible cryptographic authentication with digital patient leaflets, so a single scan proved a pack genuine and replaced the printed insert. These anti-counterfeiting solutions for pharmaceutical brands work alongside serialization and track and trace, as our pharmaceutical serialization and authentication guide explains.

  • Invisible signature at prepress: an invisible cryptographic signature was added to the packaging artwork during artwork creation. Manufacturing, printing and materials stayed exactly the same.

  • Smartphone verification: patients scan with any smartphone, through an app or the web, and get a result in under 3 seconds.

  • Real-time counterfeit intelligence: every scan records the product, time, approximate location and result. A failed check alerts the brand protection team immediately, with evidence ready to share with regulators.

  • Digital leaflets in 47 languages: after a successful scan, patients open product information in their language, which replaced printed inserts entirely. Each leaflet covers dosage and usage, side effects and interactions, precautions and contraindications, medical support contacts, and a downloadable PDF.

  • System integration: secure APIs connect to ERP, patient support and regulatory reporting systems. Anonymized scan data feeds the compliance dashboards.

Deployment

The rollout took eight weeks.

  1. Weeks 1–2: signature added to the artwork of 1,200 high-volume SKUs, the products counterfeiters target most.

  2. Weeks 3–6: extended to all 4,200 SKUs across 52 countries.

  3. Weeks 7–8: dashboards live for brand protection teams, regulators and hospital pharmacies.

There were no factory changes, no equipment investment and no increase in material costs. Everything ran through prepress artwork.
